Supplier dubious pricing, Customer purchases separate packages and supplier hides pricing combinations from customer...

Hello I have questions about dubious pricing.

Have you ever encountered a situation where a software supplier has given you the wrong pricing?
For example separate departments i.e. H.R and sales each purchase a separate package from the same supplier (IBM / Oracle /SAP/ INFORE).
If they would purchase both packages together as one product they would receive a lower price! However the supplier has the knowledge of both purchase requests and did not inform the customers and has invoiced the full price!
So what do you think is that legal?
Is it ethical?
Has this happened to your organization?
Has your organization done this to other organizations?
How can you prevent this from happening?
